a tandem jump for the passenger in accordance with the directives and methodology for tandem\njumps in the Czech Republic approved by the Civil Aviation Authority.<\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-4aa005d elementor-widget elementor-widget-heading\" data-id=\"4aa005d\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Declaration of processing the personal data: <\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-71180ca elementor-widget elementor-widget-heading\" data-id=\"71180ca\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\"><ul>\n<li>The Passenger takes due note of the fact that the Company processes his\/her personal data consisting in\nhis\/her name, surname, date of birth, weight and a visual record if it is taken, for the purpose of concluding\na contract for the provision of the tandem jump service and its execution by the Company, in order fulfil\nstatutory obligations (e.g. bookkeeping), and for the purpose of protecting its legitimate interests \u2013 in\nparticular, the recovery of receivables and the sending of marketing communications. The personal data is\naccessible to the employees of the Company and other persons cooperating with the Company, in\nparticular entities providing legal and accounting services. The passenger\u2019s personal data is processed for a\nperiod of 15 years from the provision of the service. The personal data will not be transferred to a state that\nis not a member state of the European Union or to an international organization unless it is requested by\nthe passenger or important for the performance of the contract. The passenger has the right to access\nhis\/her personal data, the right to correct the personal data, the right to its deletion, the right to the\nrestriction of its processing and portability, the right to object to the processing of his\/her personal data,\nand the right to file a complaint about the processing of his\/her personal data to the Office for Personal\nData Protection, and the passenger may exercise all these rights under the conditions laid down by\nRegulation (EU) 2016\/679 of the European Parliament and of the Council, General Data Protection\nRegulation (GDPR). Details on the processing and protection of the personal data and the rights of the\npassenger are contained in the Company\u2019s Personal Data Protection Policy, which is published on the\nwebsite: <a href=\"\/ru\/www.skydiving-prague.com\/\">www.skydiving-prague.com<\/a>;<\/li>\n<li>The Passenger also takes due note of the fact that, if he\/she fills in his\/her e-mail address and phone\nnumber above, the Company may use this information to send business communications pursuant to Act\nNo. 480\/2004 Coll. (newsletters and service offers). The passenger will always be able to unsubscribe from\nreceiving these offers from the Company. On the basis of this legitimate interest of the Company, only\noffers related to the services provided (the tandem jump) will be sent to the passenger. The passenger is\nentitled to object to this personal data processing at any time, in which case the Company shall fulfil the\npassenger\u2019s wishes. To express his\/her disagreement with this provision, the passenger must cross out this\nprovision and not fill in the contact details;<\/li>\n<li>The Passenger also declares that by filling in his\/her email address and phone number, he\/she agrees to the\nprocessing of this information for the Company\u2019s marketing purposes, i.e. offering products and services,\nincluding the sending of information about events, products and other activities, and to the sending of\nbusiness communications related to the provided services (the tandem jump), including the sending of\noffers from other entities, pursuant to Act No. 480\/2004 Coll. The Passenger hereby expressly agrees to the\nsending of business communications in the sense of the preceding sentence. A passenger declares that\nhe\/she is aware of his\/her rights, in particular that the granting of his\/her consent is voluntary, that he\/she\nmay revoke his\/her consent at any time free of charge, and that the granting of this consent is not a\ncondition for the provision of the tandem jump service. However, the revocation of this consent is without\nprejudice to the lawfulness of processing that is based on a consent granted prior to its revocation. The\nrevocation of this consent is also without prejudice to the processing of personal data processed by the\nCompany on a legal basis other than the consent. On the basis of the passenger\u2019s consent, the Company\nwill process the personal data for a period of time 5 years after the consent is granted, or until the\npassenger expresses his\/her disapproval of such processing. The passenger has the right to access his\/her\npersonal data, the right to correct the personal data, the right to its deletion, the right to the restriction of\nits processing and portability, the right to object to the processing of his\/her personal data, and the right to\nfile a complaint about the processing of his\/her personal data to the Office for Personal Data Protection, and\nthe passenger may exercise all these rights under the conditions laid down by Regulation (EU) 2016\/679 of\nthe European Parliament and of the Council, General Data Protection Regulation (GDPR). Details on the\nprocessing and protection of the personal data and the rights of the passenger are contained in the\nCompany\u2019s Personal Data Protection Policy, which is published on the website: <a href=\"\/ru\/www.skydiving-prague.com\/\">www.skydiving-prague.com<\/a>;<\/li>\n<li>The passenger agrees to the Company providing it with a link to the repository of the visual material that\nthe passenger has ordered from the Company, including the access password on the certificate, which will\nbe handed over to him\/her after the successful completion of the jump. The passenger requests that the\nvisual material be stored on the following website: <a href=\"\/ru\/www.box.com\/\">www.box.com<\/a>;<\/li>\n<li>In accordance with the provisions of Section 84 and 85 of Act No. 89\/2012 Coll., the Civil Code, the\nPassenger agrees to the capturing of his\/her person in the tandem jump for the purpose of obtaining the\nvisual material that he\/she has ordered from the Company;<\/li>\n<li>The Passenger also agrees that the Company is entitled to use the visual material (photographs and videos)\nand related audio material under the previous point capturing his\/her person for a period of 2 years from\nthe signature of this document for the Company\u2019s own promotional, business, marketing and other similar\npurposes, and the Company may do so without the prior consent of the passenger. The Company may\npublish in particular the visual material (photographs and videos) and related audio material on all its\nwebsites, on social networks (www.facebook.com, etc.), and on other Internet portals with the possibility of\npublishing the audio and visual recordings (e.g. on www.youtube.com). This consent is voluntary, and the\npassenger may revoke it at any time; this consent is not important for the performance of the contract. To\nexpress his\/her disagreement with this provision, the passenger must cross out this provision. Given that\nthe visual record constitutes personal data of the passenger, the passenger takes due note of the fact that\nhe\/she has the right to access the personal data, the right to the correction of his\/her personal data, the\nright to its deletion, the right to the restriction of its processing and portability, the right to object to the\nprocessing of his\/her personal data, and the right to file a complaint about the processing of his\/her\npersonal data to the Office for Personal Data Protection, and the passenger may exercise all these rights\nunder the conditions laid down by Regulation (EU) 2016\/679 of the European Parliament and of the Council,\nGeneral Data Protection Regulation (GDPR). The passenger declares that he\/she does not suffer in\nparticular from any illness that requires long-term care or the regular medical\nattention of a medical professional (such as heart diseases, epilepsy, circulatory\nsystem diseases, etc.);<\/li>\n<li>In the period of 24 hours immediately preceding the tandem jump, he\/she has not\nconsumed any alcohol, narcotic or psychotropic substance, or any similar substance;<\/li>\n<li>He\/she has decided to perform the tandem jump voluntarily and of his\/her own free\nwill and has carefully considered his\/her health condition before the jump;<\/li>\n<\/ul><\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-5e6980f elementor-widget elementor-widget-heading\" data-id=\"5e6980f\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Operator safety training according to SPO OM-D and tandem jump instructional \ntraining:<\/h2>\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-10af1fc elementor-widget elementor-widget-heading\" data-id=\"10af1fc\" data-element_type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\"><ul>\n<li>The client has been acquainted with and will follow the tandem skydiving\ninstructional training and the SPO operator safety traioning regarding movement at\nthe dropzone and the flight by the airborne aircraft.<\/li>\n<li>The client is expressly prohibited from taking any foreign objects on the airborne\naircraft, especially valuables such as chains, earrings, rings, bracelets, watches, as\nwell as mobile phones or other recording devices (camera).<\/li>\n<li>If you wear glasses, we recommend that you take contact lenses for the tandem\njump.
